1. That the council participating in the Integrated Care Pilot (ICP) with the NHS be approved and Lambeth Council to transform the current systems of integrated care (see paragraph 17 and 18 of this report).
2. That it be agreed to delegate to the Strategic Director of Health and Community Services the power to agree any subsequent service model decisions to be made in relation to specific ICP work streams, through the ICP Sponsor Group.
